The Past Perfect Continuous is the most unnecessarily complicated tense in the English language. It's for actions that had been going on for a period of time before another past action. Basically, it's for showing off that you understand ridiculously complex time sequences.
Subject + had been + verb-ing
Use had been for everyone - at least that's consistent!

This tense shows ongoing action → interruption/result in the past. It's like a flashback within a flashback.
